Active Duty Medical Extension (ADME), MRP, MRP2 & INCAP Pay
 
For those of you either freshly returning from a deployment, it is imperative that you know your options as they might pertain to either staying on active duty or getting out and seeking private care.
 
We are not subject matter experts on these transition options, so with that in mind, PLEASE read the WTU Guidance, dated Mar 08.  This document will detail all the options to you.

The following publications and forms are used SPECIFICALLY for INCAP pay.
 
AR 135-381, Incapacitation of Reserve Component Soldiers
 
DA Pam 135-381, "How-To" manual for applying for INCAP pay

 
The following forms are used when applying for INCAP pay.
 
Fill out this one first, and get your commander to sign and date
DA Form 7574-2, Soldiers acknowledgement of INCAP pay counseling
 
Have your military physician complete this form.
DA Form 7574-1, Military physician and fit for duty statement
 
You and your commander must complete this form.
DA Form 7574, Monthly Claim form for INCAP pay
